dc.description.abstractThe medical home plays a critical role on the interprofessional team to improve school readiness of children before they enter primary school. School readiness and optimal health require a combination of medical care with health and social services both to prevent and to improve the effects of negative conditions. Hawaii Medical Association has two new projects aimed toward that end.
